This page records consents you have given your club of use of your personal information including photographs or video.

Your club can log consent by uploading a signed proof of consent by you which is visible by clicking the image in the image column (if present).

You can revoke a consent at any time by clicking the button in the active column. The status will change to Revoked. If you change your mind it can be reactivated.

Adding Consents

You can grant consent by clicking Add New Consent

Select the consent to be granted (controlled by your club) and what you are consenting to will appear. This cannot be changed later by your club.

Enter the date granted and click Add to Consents

As you are giving consent there is no need to upload proof but you can if you wish (photo or PDF).
